Return/Exchange Policy
To return or exchange product(s), please follow the procedures below within 5 days of receipt:

1. Call Diabetes Supply Center at (402) 399-8444 (in Omaha) or (800) 779-3374 (outside Omaha) to obtain a Return Goods Authorization Number (RGA#). This number must be written on the outside of return packages as described below.
    
2. Return product(s) to Diabetes Supply Center, RGA# _____, 2910 South 84th Street, Omaha, NE 68124, with:
   • Original invoice indicating your daytime phone number
   • RGA# written OUTSIDE of return package as indicated above
   • Product(s) to return/exchange in original packaging
    

After receipt of return/exchange, it will be inspected prior to approval of return/exchange. If product(s) have been used or damaged, return/exchange will not be approved. We will notify you if return/exchange is not approved.
